What mainboard do you have?
You can use HWMonitor (or any other similar prog like Everest) to check temperatures and more.
Open CCC and check the performance tab. AMD Overdrive active? Did you ever overclocked or underclocked your GPU/CPU? (in Bios mainly)
Otherwise uninstall your current driver (and everything related) and get the latest from amd.com
It's also a good idea to "play" around with your hardware, like using a different PCIE slot for your GPU if possible, detach and attach everything (cables etc) and simply cleaning your computer inside (turn off the power supply first and press the on/off button of your PC to get rid of the remaining electricity)
/edit: You should open the Windows Event Viewer and check the different protocols (mainly System and hardware) for Warnings and Errors. With some luck it could give use something interesting.
