Marx was largely considered kind of a harmless kook by most (Look at his personal life, he was basically a tendy eating manchild for the 19th century). It was Lenin who really ran with it, and had the political backing and force of personality to start the communist revolution. Without Lenin's successes, none of the rest of the pro communist movements (The USSR, CCP, South America) really take off.

Except that isn't a symptom of Tourette's, at least not explicitly. Tourette's requires at least one motor and one physical tic with some qualifiers like time. The swearing is not a particularly common tic amongst Tourette's sufferers, and there isn't any evidence pointing to an internal association. Most of the time it's some combination of swearing and nonsense, like "*** my broom with your dog ***swaggler".
